INTAKE PACKET
At your request an intake packet will be sent to your email. This is a digitized format, which serves as an Introduction to me and enables me to understand you and your issues going into scheduling an Intake Visit. Please fill out the materials and the assessment measures and read and agree to the Informed Consent. The packet is confidential and will not be shared save possibly with another practitioner who is my practice and who is also pledged to your confidentiality.
MEDICAL RECORDS
Please bring copies of any medical records, prescriptions, diagnoses you may wish to present. I will retain these for my records. I am not able to copy your materials at the office(s).
PAYMENT
Payment may be by check, cash or credit card and is due at the time of service.
Presently, ketamine is not covered by insurance, except for Spravato—which is a very different approach and not one we provide or support. Psychotherapy which is a significant part of your experience with me is potentially reimbursable to you depending on your insurance and its coverage for an outside provider. I will do my best to help you receive reimbursement if this is possible.Reduction in fees may be discussed at the Intake appointment whose fee is fixed. A reduction in fee depends on my, or my colleague’s openness to do so, and is not automatic. For what we provide in service to you, we are the most reasonable of practices of which we are aware.
